Breast Augmentation

Breast augmentation is a surgical procedure, performed to enlarge the size of a woman’s breasts. Apart from improving the body contour, breast augmentation will greatly help in boosting one’s confidence and self-esteem. Nowadays, thousands of women pursue breast augmentation procedures for a variety of reasons. Most women opt to have breast augmentation surgery to enhance their figure and achieve their ideal body image, to balance disproportionately sized or shaped breasts, or to correct a reduction in breast volume caused by significant weight loss, pregnancy, age, breast feeding, and other factors. Breast enhancement can also be done to reconstruct the breast for those who have undergone a mastectomy surgery.

Breast Augmentation Using Surgical Implants

Known also as augmentation mammaplasty, breast augmentation involves the use of surgical implants. Primarily, two types of breast implantsare available to patients: saline-filled and silicone-gel-filled implants. Both these implants are FDA approved and made of solid silicone plastic. Silicone implants look and feel more natural than saline implants, as these have a texture similar to that of breast tissue – this is one of the significant advantages of silicone breast implants.

Breast implants can be placed in a variety of positions, either through an incision in the armpit, at the edge of the areola, or under the breast. For saline implants, the incision is about an inch long, whereas the incision is an inch and a half long in the case silicone implants. Factors that influence incision location include age, implant type, areolar diameter, current body mass, and breast anatomy. Axillary incision is best suited for women having little breast tissue and small areolae. It is better to make the incision either under the breast or at the edge of the areola, in the case of women with breasts that were previously larger, but which have shrunk following childbirth and breast-feeding.

The implant may be placed in a pocket, either beneath the pectoral muscle, directly at the back of the breast tissue, or above the pectoral muscle. This would depend on aspects such as the implant option, individual physical characteristics, and extent of augmentation needed.

Breast Implants in a Variety of Sizes and Shapes

To accommodate various body types, breast implants come in a variety of sizes and shapes. Implant sizes range from: 100 to 400 cc. Apart from individual preferences, size selection is influenced by one’s chest size, height, and the amount of breast tissue present. The shapes of breast implants available are tear-drop and round. Implant with a rough (textured) or smooth surface is also another option. The risk of capsular contracture is less with a moderately rough surface. Textured implants, particularly saline types, seem to wrinkle more than smooth ones. Smooth implants are favored for all applications.

The two complications associated with the use of implants are rupturing and capsular contracture. Silicone implants might eventually rupture and the gel leak to the scar capsule and into the bordering tissues, which can go completely unnoticed for many years. When a silicone implant ruptures, the shape and size of the breast remains unchanged, which makes it hard to detect. If this happens, the gel can be removed without causing any harm. You will require frequent MRI monitoring to detect early silent rupture.

In the case of saline implants, rupturing is easier to detect. The body will absorb the fluid when rupture or leakage occurs; causing decrease in breast size that can be easily noticed. The saline fluid leaking out can be removed through the kidneys. Both saline and silicone implants eventually require replacement. Saline implants are built to last for ten years and more.

The body will always form a capsule of scar tissue (collagen fibers) around any foreign substance placed under the skin. And this capsule can become contracted and tightened around the implant. This can lead to pain and may alter the appearance of the implant and cause hardening of the breast, which may necessitate removal of scar capsule or implant replacement.

Other possible complications that are likely to occur are infection, bleeding, fluid accumulation, scarring, persistent pain, and short-term or permanent changes in breast or nipple sensation. Additional surgery may be required if problems such as infection or bleeding occur. However, before undergoing the surgery, make sure to ask your plastic surgeon all your doubts concerning the procedure. You will also want to discuss in detail the pros and cons of the two implant options, and which one suits you best.
